Referrals
If you are in need of a physician referral or pre-certification for a test, please notify us as soon as possible. If a follow-up visit is required, our staff will provide you with the physician's name and contact information. It is your responsibility to make the appointment and call us with the appointment date. All pre-certification tests will be scheduled at the front desk.
Payment Policy
We accept cash and credit cards (Mastercard and Visa). If you are enrolled in a health plan or would like to be seen outside of your plan, payment is required at the time of your visit.
