Appointments
To schedule an appointment call 770-506-0100. Please notify us at least 24 hours in advance if you know you need to cancel an appointment. There is a $25 charge for appointments that are not cancelled 24 hours in advance. If you arrive late for an appointment, it may be necessary to reschedule. We strive to provide quality service to our patients by honoring scheduled appointment times.

Referral Procedures
If you are in need of a physician referral or pre-certification for a test, please notify us as soon as possible. If a follow-up visit is required, our staff will provide you with the physician's name and contact information. It is your responsibility to make the appointment and call us with the appointment date. All pre-certification tests will be scheduled at the front desk.

Insurance
We accept most major medical plans. However, it is important to contact the office to confirm your plan's participation. If you have insurance you MUST present your insurance card at the time of your visit. Please notify the office of any address, phone number or insurance changes. Co-payments are due at the time of your visit.

Payment Policy
We accept cash, check, Mastercard and Visa. There is a service charge of $25 on all returned checks. If you are enrolled in a health plan or would like to be seen outside of your plan, payment is required at the time of your visit.
